All required forms will be sent to you securely through our SimplePractice client portal once we’ve confirmed that you’re ready to begin care.

These digital forms can be completed and signed online before your first session. They include:

  • Informed Consent & Professional Disclosure
  • Court & Legal Service Fees
  • Credit Card Authorization Form
  • Demographics & Intake Questionnaire

There’s no need to download or print anything—we’ll walk you through it all during the intake process.
