New Client Paperwork

All required forms will be sent to you securely through our SimplePractice client portal once we’ve confirmed that you’re ready to begin care.

These digital forms can be completed and signed online before your first session. They include:

  • Intake & Demographic Information
  • Informed Consent & Privacy Practices
  • Telehealth Consent
  • Financial Agreement
  • Optional: Release of Information (if you’d like us to coordinate care with another provider)

There’s no need to download or print anything—we’ll walk you through it all during the intake process.
